RELEASE AND HOLD HARMLESS AGREEMENT
My child has voluntarily agreed, with my permission, to engage in recreational activities, including baseball, softball, weight training or any other sport or activity at the business of Spring Mills, LLC, dba Diamond Sports Training, located at 134 W. Streamside Place, Spring Mills, PA. I acknowledge that my child’s participation in this activity is completely voluntary. Further, I agree to fully remise, release, quitclaim and forever discharge, fully indemnify, defend, hold harmless Spring Mills, LLC, dba Diamond Sports Training from and against any and all claims, demands, damages, losses and expenses, including attorney’s fees, arising out of or resulting from any injury or loss sustained by my son/daughter while playing baseball, softball, weight training or any other sport or activity at the location known as Spring Mills, LLC, dba Diamond Sports Training, located at 134 W. Streamside Place, Spring Mills, PA 16875.
Further, we do hereby waive, release and forever discharge said organization, its staff, officers, agents, representatives, employees and their successors and assigns from any and all claims for damages occurring from an accident, injury to person, or loss of personal property occurring during their participation in voluntary activities at the location.
